Who Is Eligible?
All full-time team members who have been full-time for at least 12 months are eligible.
What Steps Should You Follow?
If you would like to be considered for a cosmetic procedure, the first thing you need to do is get approval from your team leader. Once approved, a consultation must be schedule through the Patient Care Coordinators.
What To Expect
If you are determined a candidate, you will be provided a quote at full cost for the procedure, as given to all standard patients.
You will also receive a discounted quote, in which the surgeon’s fees, OR fees, supplies, and anesthesia will be priced at a discounted rate based on your work schedule.
The surgery and pre-op appointment will be scheduled by the Patient Care Coordinator, who will confirm with your team leader that the timing is reasonable.
Team Member Cost
Under this benefit, the amount you will pay $500 per scheduled hour of surgery.
Time Off for Procedure and Recovery
You will need to use your Paid Time Off, which includes getting approval from your leader on timing.
Any necessary revisions you may need within 12 months of surgery will be provided at no cost.
Any team members getting surgery agree to meet with the Marketing Team within 3 days of their initial consultation to determine the best way to share their story in exchange for the discount.
If You Leave the Company
If you voluntarily leave the practice within 12 months of having your surgical procedure, you agree to pay the pro-rated amount of the full/standard surgical quote based on the number of months remaining before 12-month endpoint—less the discounted amount paid prior to surgery.
